i think it has more to do with the inability to stop and take action. you always hear very overweight people talking about they wish they could get the motivation or want to start at a given date... they just never do and it keeps adding up.
i guess its the same way depression is. you know you need to take action but you never actually take action. until you have some sort of intervention you will continue down the spiral.
