Out the window for whom? Doctors? Rx writing based on profit margin to the Doc rather than best benefit to patient is crap if that's what you were getting at. I have seen (been in) chop shops like that. Basically pill dispenseries. Frankly I think the AMA should shut those fuckers down.

The bonus etc would be part of the gov't plan to reimburse or would this be an attempt to institute a program like this w/private carriers too? It sounds like, especially the p4p, a way to reduce costs and improve quality of care. That's hard to take issue with.

lol - then they would have to shut every clinic down in the us. i think you are missing what i am trying to say. physicians always made their money on their drug expenditure until the MMA. the MMA, with the implementation of ASP+6%, has caused a lot of drugs to be reimbursed at a lower rate than their cost so physicians simply do not use those items or send those cases to the hospital. they are a business and no business stays afloat losing money. "consignment drugs" would take that issue off of the table.

p4p is already in use regarding only cms payments and the programs are growing annually. the insurance carriers are not in on that at this point and i have not heard any inkling that they are going that direction. hopefully they are at some point because it shows quality of care based on actual evidence.

ok - so then whose profit motive are you tossing out the window?

And, I have no problem with doctors making money - as you say its a business. My complaint was doctor looking at billing issues ahead of patient's needs.

Hell, add in people with terminal illnesses and those with very low odds of recovery. End of lif care is astronomically expensive so the savings should cover a lot of other things.
