Yes, cms is a looming nightmare... especially with the accelerating rate that the boomers will start retiring.

CMS are the only federal progams but states like Masachusettes and Oregon have state systems in place that can be looked at to see what works or doesn't. I'd guess there are others - those are the only two I'm aware of off the top of my head.
